Please note: This RMA Request Form is only intended for use if you purchased your urn through this website. If you purchased your item from a Funeral Home or Jewelry Retailer, please return the item to where you purchased it, and they will handle your Warranty claim or contact us directly by the phone or email listed above.

Important Note: Do not ship your item back before completing this form and receiving a Return Merchandise Authorization and official shipping information from Pacific Urns. We cannot be responsible for shipments that do not follow our processes and improper shipping may impact our ability to refund you.
